On behalf of the Sisters of Mercy and everyone at Emory Saint Joseph’s Hospital (ESJH), thank you for entrusting us with your health care needs. As Atlanta’s first faith-based and longest-serving hospital, our mission is the same today as it was over 140 years ago—to provide compassionate care, especially to those in need. We understand the stress that being in a hospital may create for our patients and their families. We also recognize that planning your care for after you leave the hospital can be concerning. Our Care Coordination team consists of nurse case managers and social workers here to assist with your discharge planning. Along with your physicians, our Care Coordination team will work closely with you to keep you informed about your treatments, medications, and resources available when you leave the hospital.
